Diabetes Screenings

Course Description

The American Diabetes Association recommends testing for everyone age 45 and up. Diabetes checks are available in the Diabetes Services offices at St. Nicholas Hospital and Plymouth Medical Plaza, by appointment only. There is no charge. To make an appointment, please call 920-459-5192 (St. Nicholas) or 920-892-2576 (Plymouth Medical Plaza). Screenings are also available without an appointment during scheduled times at Community Bank, Taylor Drive, Sheboygan.
