Home Health is a state-licensed, Medicare certified, and Joint Commission accredited department of St. Nicholas Hospital. Home Health offers services to individuals in their home who require the skills of a nurse or therapist, are homebound, and are under the care of a physician. Home Health services focus on improving the health and independence of those in the community while allowing individuals to receive healthcare services in the comfort of their own home. Home Health offers a variety of services at a significant cost savings and Home Health is covered by Medicare, Medicaid, and most insurance plans.
Home Health is available across the life span for individuals recovering from serious illness or injury to those suffering from a chronic condition. From newborns with sleep apnea to an adolescent or adult diagnosed with diabetes, or an elderly person recovering after a fractured hip, just to name a few, home health is available to assist individuals in maintaining or returning to an optimal level of health and functioning.
The St. Nicholas Hospital Home Health Department is also the first home care agency in Sheboygan County to offer electronic and digital oversight of wound care patients by a satellite site and credentialed specialties.
Telehealth is the delivery of health-related services and information via telecommunications technologies. It is as simple as two health professionals discussing a case over the telephone, or as sophisticated as using videoconferencing between providers at facilities in two countries.
Telehealth encompasses preventive, promotive, and curative aspects. The format of telehealth that St. Nicholas Hospital will employ is called store-and-forward telehealth. Digital images, video, audio, and clinical data are captured and "stored" on the field nurse's computer; then transmitted securely to the St. Nicholas Hospital Comprehensive Wound Care Center where they are studied by specialists and their input is provided.
Home Health services through St. Nicholas Hospital Home Health & Hospice
are available within a 30 mile radius of St. Nicholas Hospital. Referrals to Home Health can be made by physicians, families, or a prospective client him/herself. A specialized home health Liaison will work with individuals to determine eligibility and arrange for services. Care is available on an intermittent basis, 24 hours a day, seven days a week and a Registered Nurse is only a phone call away around the clock for those admitted into services. Home Health Services are supported in part by the United Way and in keeping with the mission, vision, and values of St. Nicholas Hospital; no one will be turned away regardless of the ability to pay.
