FOR IMMEDIATE RELEASE St. Nicholas Hospital Athletic Health Center Offers Physical Exams Sheboygan, Wis.The St. Nicholas Hospital Athletic Health Center will be offering physical exams for North High School , South High School , Oostburg High School , Sheboygan County Christian High School, and Sheboygan Lutheran High School students on Monday, May 24, 2010 and Wednesday, May 26, 2010, at the Hospital. The comprehensive pre-participation evaluation includes a medical evaluation, blood pressure and vision screening, orthopaedic screening and consult if necessary, injury history review/previous injury re-evaluation, flexibility/strength assessment, and posture assessment. The physical exam is $20 per person and is due at the time of registration. Cash will be accepted or checks can be made payable to St. Nicholas Hospital. Insurance billing will not be billed. Proceeds from the physical exams will be given to the high school athletic departments. Students should allow 60 minutes for the physical exam. The physical exams are conducted to fulfill WIAA athletic participation requirements and meet WIAA physical exam requirements. Summer camp forms will not be signed. Students are asked to wear the following clothing to the physical exam: Males: Athletic shoes, shorts, and a t-shirt. Students are asked to bring their WIAA physical card and a health history form which has been signed and completed by a parent or legal guardian.
