  • Care provided within a 30-mile radius of HSHS St. Nicholas Hospital, providing us with the ability to respond promptly to your needs. 
  • Pharmacists who work directly with hospice nursing and are on call 24 hours a day.
  • Direct-admit inpatient hospice available at HSHS St. Nicholas Hospital. No emergency department visit is required.
  • An elite team of nurses who maintain the continuity of care for our patients. 
  • Providing compassionate care for more than 20 years. 
  • Bereavement follow-up care offered following a patient's death.
Your care team includes the following professionals:
  • Attending physicians who oversees patient care and manages symptoms.
  • Registered nurses who assess the patient's ongoing health needs. 
  • Licensed practical nurses that provide care under the direction of a registered nurse. 
  • Social workers who provide emotional support, while respecting personal beliefs.
  • Trained volunteers who provide companionship, caregiver respite and transportation assistance.

Who is eligible for Hospice?

Hospice care is for those who: 
  • Have a life-limiting illness
  • Are expected to live six months or less
  • Are seeking comfort care and not curative treatment
